Individual Teller Acuity Cards®(TAC) replacement cards are now available to customers who have previously purchased a full or half set. In order to proceed to checkout, a photo of your damaged card must be uploaded.

This test has extensive evidence based on normative and clinical studies that provide a strong foundation for TAC applications in ophthalmology, optometry, and developmental practice. The full set consists of all sixteen cards, each of which has an approximately 4-mm diameter peephole at the center and includes a Blank and gratings from .23 to 26.0 cycles/cm and the half set includes 8 cards ranging from .23 to 26.0 cycles/cm.
