The Precision Vision 2140-66 Sloan Letters ETDRS Near Vision Card is a double-sided logarithmic visual acuity chart designed for standardized near and intermediate visual acuity testing at 40 cm (16 in) and 66 cm (26 in).

The card incorporates Sloan letter optotypes arranged in scrambled sequences derived from the ETDRS Original and ETDRS 2000 series. Each side is calibrated for its specified testing distance and uses 0.1 log unit progression between acuity levels, supporting consistent letter-by-letter visual acuity assessment.

Chart 1 is calibrated for testing at 66 cm (26 in) and provides an extended range from approximately 20/250 to 20/6.3. Chart 2 is calibrated for testing at 40 cm (16 in) and ranges from 20/400 to 20/10.

An attached measuring cord with three positioning balls accommodates both the 40 cm and 66 cm testing distances, helping the examiner establish and maintain the appropriate chart-to-patient distance during testing.

The chart displays multiple visual acuity notations alongside each line, including M-unit letter size, logMAR, decimal acuity, 6-meter Snellen equivalent, and 20-foot Snellen equivalent, allowing results to be interpreted and documented using commonly utilized acuity scales.
